A.) Care Management is an all encompassing care model, looking into each aspect of a person’s life to see what support systems they may need, all while tailoring a care plan to their individual lifestyles. The unique tailoring care planning process helps to provide an accurate evaluation and assessment to allow Cook Care Management to provide knowledge, resources and support based on the individual client. Years in the field opened Trish’s eyes to ongoing issues aging individuals face everyday. So many members of the aging community were being talked over or down to, ignored, left alone, dismissed, neglected, and were not being treated with the level of compassion, respect, or dignity they deserved. The “ That is normal for someone of your age” or “ That is just apart of aging” stigma needed a new perspective and approach. No two individuals are the same nor do they age the same. So no two care plans should be the same either. Cook Care Management Care customizes care plans to fit the individual...not the entire aging community and it is done so with the utmost compassion, respect, trust, support and always with the client’s well-being and preferences in mind.
Q.) What does an Geriatric Care Manager do?
A.) An Aging Life Care Professional, also known as a geriatric care manager, is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. As Aging Life Care Managers we help our clients and their families locate and coordinate resources. We pride ourselves on our ability to work cohesively with all care providers near and far. Some of these providers include: Doctors, pharmacies, family, friends, home care agencies, senior communities, and many more. Cook Care Management works hard to create the absolute best care plan possible and adjusts it as aging issues occur. Our clients become an extension of our family. A.) While insurance is not a required piece to owning a care management company, Cook Care Management does choose to hold general liability and professional liability insurance for each employee. The owner of the company also has an auto insurance policy that would allow her to provide some transportation as needed.
